Ontario Health West Referrals

Referrals can be made within the agency or to community partners. If you are homeless or at a great risk of homelessness, several hospital stays or severe mental health diagnosis, you qualify.

Complete a referral from Mental Health and Addictions Access Line, then complete an intake at Gateway in order to be considered for various programs.
